This package offers training on how to conduct and write a deviation investigation and associated root cause analysis. Materials include two presentations, which go over formal investigation methods (e.g. Fishbone Diagrams, Five Why's exercises) as well as writing style tips for formal investigation reports. Examples of good and poor investigation reports are given in order to illustrate the topics covered by the presentations. Also included is a checklist which summarizes the content, and provides a handy reference for future investigations that will occur.
